This section contains all the forms that are required to be completed by different parties at various stages and context of the dispute resolution. If you are unsure which form you need to use, please contact us for assistance.
Resolution Facilitation: This service is available to obtain the assistance of a neutral third-party to resolve a dispute, upon consent of all parties involved. The form must be completed and signed by all parties to the dispute. Charges will apply if the concerned sport organization is not federally-funded.
Ordinary Tribunal: The Ordinary Tribunal deals with appeals arising from the application of appeal policies of sport organizations. Team selection and carding disputes, among others, fall under this category. Charges will apply if the concerned sport organization is not federally-funded.

Doping Tribunal: The Doping Tribunal deals with matters arising from the application of the Canadian Anti-Doping Program only.

Request for a Hearing - Doping Tribunal
Safeguarding Tribunal: This division only deals with matters arising from code of conduct violations, for which the concerned sport organization has an agreement with the SDRCC to conduct hearings. Please contact our case managers to obtain the relevant forms.
Appeal Tribunal: The Appeal Tribunal exclusively hears appeals from decisions rendered by the Doping Tribunal or the Safeguarding Tribunal. Please contact our case managers to initiate an appeal from the Safeguarding Tribunal.
